The 506 Form: What Indian Parents Need to Know, 1977-09-12

This letter from the Office of Indian Education explains the new 506 Form and why it is necessary. It also states that once the form is filled out for a particular child, it will stay on file with the school district, so parents will not need to fill out another form for that child as long as the child attends any school in the district.
