The Cherokee Tribe of Oklahoma requires applicants for membership to submit a Certificate of Degree of Indian Blood (CDIB) from the Bureau of Indian Affairs (BIA), along with a Standard Birth Certificate or Delayed Birth Certificate. If submitting a Delayed Birth Certificate, an additional document such as an Affidavit of Personal Knowledge and Memory signed by an older relative or friend that knew of the applicant's birth will be required. Birth or death certificates for all enrollee descendants that the applicant is tracing through must also be submitted. The BIA will issue a CDIB to qualified applicants, after which the Cherokee Tribe of Oklahoma will issue a blue tribal membership card.
